Chronic Disease Innovation Fund

The Chronic Disease Innovation Fund is a $300,000 annual fund designed to address the root causes of chronic disease.

Community groups, working in partnership with community health boards, will propose projects that will help Nova Scotians live longer and healthier lives. These will include helping people stop smoking and reduce their alcohol consumption; improve exercise levels; helping people eat more fruit and vegetables; and combatting obesity. Up to $75,000 will be available in each health zone of the province.

What is the deadline?

What do I have to demonstrate in my application?
Please read the guidelines and criteria carefully when completing your application.

The application must include:

  • A letter of support from the CHB(s) and partnering organization(s)
  • Project summary (150 words)
  • Project overview (1000 words)
  • Description of how the proposal is innovative (200 words)
  • Expected project outcomes and evaluation plan (400 words)
  • How the project can be operationalized within 60 days of notification of funding, and completed within the 1-3 year time period (250 words)
  • An identified project lead
  • Contact information of the applicants

When will I hear about the grants?
Successful applicants will be notified the end of October.

When must the projects be up and running?
Projects must be operational within 60 days of notification.

Do we need to report back on our progress during the project?
Yes, for this year projects require one interim report (February 15, 2017) and an end of year report (March 31, 2017).

The Innovation Fund will support multi -year projects, so deliverables for the fiscal year must be achieved before subsequent funding for the next fiscal is awarded. Twenty five percent (25%) of the award is withheld until the final report is received at project end.

Can I apply for multi-year funding?
Yes, project deliverables for each year must be met before subsequent fiscal funding will be provided.

What needs to be included in the application to outline the costs for the project?

An operational budget and budget justification must accompany each application. Please identify the budget that will be needed for each of the following (as applicable):

  1. Project start-up costs
  2. Project budget ending March 31, 2017
  3. Budget for 2017/18 fiscal year for projects that will continue past March 31, 2017
