Please complete this Fire Report Form as completely and accurately as possible. The information obtained is used by the Office of the Fire Marshal in its reporting process and for the development of training and education programs. The data is also submitted for use in the Canadian Fire Statistics Database.

All fields displaying an asterisk (*) must be completed for the form to be accepted. Also, please note the time fields are in 24hr format. i.e. 1:00pm = 13:00
