Submit MSI forms
You can also submit the following MSI and Pharmacare forms (and any supporting documents) online. Visit the form start page and complete the form. Once you complete a form, click on the 'Start now' button. If you're applying for a Health Card, you don't need an MSI Online account to apply.

More about MSI Online
MSI Online is available to persons who have a Health Card and are a Canadian citizen or Permanent Resident and are 19 or older (exceptions may apply to 16 or older).
To access your MSI Online account, you need to first sign into your My NS Account (you need to create a My NS Account if you don’t already have one). Once you sign in, you’re redirected back to MSI Online.
If you create a My NS Account, you need to know your Health Card number.
