Record and register form
This form is used to support your application for Employment Support and Income Assistance (ESIA). Completing it before you contact Income Assistance can help staff prepare and provide better support.
If you're able to, you can complete the form and bring a printed copy to your local office. Interpretive services are available at each office.

Confirmation of mailing address
This form is used to set up a mailing address to receive Employment Support and Income Assistance letters when you do not have a place to live. Please fill it out when a caseworker requests it.
