Collaborative practice teams can include doctors, nurses, social workers, mental health professionals and others working together to get Nova Scotians the care they need.
This team approach improves access to care by putting patients in front of the right health professionals at the right time. The result is shorter wait times and less frustration and confusion about where to go next.
There are approximately 50 collaborative practice team locations in the province, in various stages of development, supported by Nova Scotia Health Authority and the Department of Health and Wellness.
