If you are selected for an interview a doctor will have to complete the Certification of Medical Fitness for Correctional Officer/Youth Worker Form. The Certification of Medical Fitness for Correctional Officer/Youth Worker provides assessment information used to assist in identifying candidates who meet the physical and mental health requirements necessary to perform the duties of the position.

Candidates must print off the attached form and bring it in a sealed envelope to the interview.
