Joint Service and Access Policy (Pharmacy Software Vendors):

The purpose of this policy is to:

• define the mutual responsibilities of the Drug Information System Program and Pharmacy Software Vendors and ensure they are aware of the rules associated with accessing and providing access to the Drug Information System
• assist in the protection of privacy with respect to the personal health information collected, used, disclosed, and retained in the Drug Information System

Schedule C – Confidentiality Agreement

This Schedule addresses the privacy and security of any Drug Information System proprietary information or personal health information and verifies that the signator has read the Joint Service and Access Policy document. Signature of this Schedule confirms agreement with confidentiality requirements.
